29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:     a.  On or about April 3, 2019, in the production area, employees were required to operate the Verson Allsteel press brake #13 on a daily basis. The employer failed to provide adequate guarding for the Verson Allsteel press brake #13. Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.    b.  On or about April 3, 2019, in the production area, employees were required to operate the Heim press brake on a daily basis. The employer failed to provide adequate guarding for the Heim press brake. Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.     a.  On or about April 3, 2019, in the production area, employees were required to operate the Verson press brake #2 on a daily basis. The employer failed to provide adequate guarding for the Verson press brake #2. Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.

On or about April 17, 2019, in the production area, Federal Press #7 had an uncovered foot pedal. Employees were thereby exposed to the hazards associated with unexpected machine operation.      a. On or about April 3, 2019, in the production area, Federal Press #8 (notcher) had inadequate guarding that would allow an employee's hand to reach the point of operation on the operator's side of the press. On or about April 3, 2019, in the production area, the Clinton #5 press did not have a guard affixed to the inside of the flywheel. Employees were thereby exposed to the hazards associated with rotating parts.
