29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.  On March 28, 2019, employees worked on a job-made scaffold at this residential, new home, construction site. The walking working surface was less than 18 inches wide. Employees were exposed to fall hazards.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level, and was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On March 28, 2019, employees worked on a job-made scaffold at this residential, new home, construction site. The scaffold was more than 10 feet (3.1 m) above lower level, and employees were not protected from falls by the use of personal fall arrest systems or guardrail systems.
