29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) B & B Service, LLC located at 103 Harper Street, Louisville, CO 80027: On or before April 2, 2019, the employer did not ensure that employees were protected by hard hats while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects.  The employees working on the upper levels were throwing shingles that contained nails over areas where other employees were working without hard hats.  This condition exposed employees to struck-by hazards from falling shingles.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) B & B Service, LLC located at 103 Harper Street, Louisville, CO 80027: On and before April 2, 2019, the employer did not ensure that employees were provided with and used safety glasses when using pneumatic nail guns powered by 120 psi.  Employees were installing shingles on a single story ranch using pneumatic nail guns.  Employees were not provided with or told that they needed to use eye protection while using the nail guns.  This condition exposed employees to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) B & B Service, LLC located at 103 Harper Street, Louisville, CO 80027: On or before April 2, 2019,  the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with alternative fall protection measures.  Employees were installing shingles on the roof of a single family ranch which was up to 15 feet at the ridge without the use of fall protection.  The exposed employees to falls from height.  B & B Service, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1283358, citation number 1, Item 2, issued January 19, 2018 and became a final order on January 30, 2018, with respect to a workplace located at 3335 Benton St, Wheat Ridge, CO 80212

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  a) B & B Service, LLC located at 103 Harper Street, Louisville, CO 80027: On and before April 2, 2019, the employer did not ensure that employees were provided with and used adequate personal protective equipment while performing roofing work.  Employees were using pneumatic nail guns powered by 120 psi and were not provided with safety glasses.  Employees were performing work involving overhead hazards, and were not provided with hard hats.  These conditions exposed employees to struck-by hazards.
