29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift. On or about December 11, 2018, employees were exposed to a fall of approximately 15 feet while performing tree removal activities on a residential home located at 1128 Columbia, Valley City, Ohio 44280. The employees were working from an articulating aerial lift and the employer did not ensure they were using fall protection.
