On or about April 3, 2019, employees were exposed to a fall of approximately 12 feet while installing roofing materials on a new residential home located at 1258 Ledgeview, Hinckley, Ohio 44233. The employees were working on all areas of the roof and the employer did not ensure they were using fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about April 3, 2019, employees were exposed to a fall of approximately 12 feet while installing roofing materials on a new residential home located at 1258 Ledgeview, Hinckley, Ohio 44233. The employees were working on all areas of the roof and the employer did not ensure the employees were trained on how and when to use fall protection.
