29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a.  On or about April 3, 2019, at the above addressed jobsite, employees conducting masonry operations from a  tubular welded frame scaffold were not trained to recognize the hazards associated with scaffolding.  The employees were thereby exposed to the hazards associated with falls.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch wide.       a.  On or about April 3, 2019, at the above addressed jobsite, employees were exposed to falls while performing masonry operations from a tubular welded frame scaffold, twenty-four (24') feet above the ground, that did not have the working level fully planked.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews.    a.  On or about,  April 3, 2019, at the above addressed jobsite, employees conducting masonry operations from a tubular welded framed scaffold, twenty-four (24') feet above the ground, were exposed to fall hazards due to the lack of a guardrail system at the end of the outrigger platform.
