29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to the hazard of being assaulted by patients: (a) Emergency Department: Acts of physical assault and workplace violence were caused by patient aggression during medical evaluations and examinations, workers were not provided with an efficient way to alert others or call for help. (b) Community Living Center: Acts of physical assault and workplace violence were caused by patient aggression during medical evaluations, examinations, treatment and care, workers were not provided with an efficient way to alert others or call for help. (c) Acute Care Areas: Acts of physical assault and workplace violence were caused by patient aggression during medical evaluations, examinations, treatment and care, workers were not provided with an efficient way to alert others or call for help. (d) Facility Wide: Acts of physical assault and workplace violence were caused by patient aggression during medical evaluations, examinations, treatment and care. There was no functioning mechanism for timely notification or alert of a patient's previous violent actions against employees.
