29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a) IMC Framing, Inc. located at 4449 Raleigh Street, Denver, CO 80212:  On and preceding 4 April, 2019, the employer did not ensure that employees on walking/working surfaces were protected from falling through holes more than six feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  A new construction residential home had two floor holes measuring approximately 3 x 4 feet that opened to a drop of approximately 9 feet to the basement floor.  The holes were not covered nor was a guardrail or other means of fall protection used by the framers working in the direct vicinity of the holes.  This condition exposed employees to fall hazards from height.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) IMC Framing, Inc. located at 4449 Raleigh Street, Denver, CO 80212:  On and preceding 4 April, 2019, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, or personal fall arrest systems.  Framers were installing floor joists approximately 10 feet above ground level without the use of a personal fall arrest or other fall protection system.  This condition exposed the employees to fall hazards from height.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) IMC Framing, Inc. located at 4449 Raleigh Street, Denver, CO 80212:  On and preceding 4 April, 2019, the employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Framers were installing floor joists approximately 10 feet above ground level without the use of a personal fall arrest or other fall protection system.  The employees were not trained on safe work procedures, such as the use of retractable lanyards, to limit the employees fall hazards.  This condition exposed the employees to fall hazards from height.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    a)  IMC Framing, Inc. located at 4449 Raleigh Street, Denver, CO 80212:  On and preceding April 4, 2019, the employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause harm to employees utilizing electrical outlets and flexible cords.  Several damaged pieces of electrical equipment which includes but is not limited to equipment power cords, power outlets, and extension cords were being used at the site. The equipment was used to power tools which include air compressors and circular saws. The following condition exposed employees to shock hazards.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    a) IMC Framing, Inc. located at 4449 Raleigh Street, Denver, CO 80212:  On and preceding 4 April, 2019, the employer did not ensure that an evaluation of each powered industrial truck operator's performance was being conducted at least once every three years.  A JLS 6042 rough terrain forklift was operated at this site to move materials throughout the job.  The rough terrain forklift operator last received forklift operator training in February of 2010, and for which the certification card said that this training had expired in 2013.  This condition exposed employees to struck-by hazards.
