29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Jobsite, 7330 McClure Avenue, Pittsburgh, PA:  On or about April 4, 2019 the employer had not provided training to employees on potential fall hazards and what methods could be used to eliminate or control such hazards.  Employees were exposed to fall hazards from the roof area where they were constructing a dormer and inside the structure where open sided floors were unprotected.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  (a) Facility, 544 Fifth Street Ext., Trafford, PA:  On or about April 4, 2019 the employer had not provided training to employees on the use of ladders to ensure they could recognize hazards associated with ladder use and what methods should be used to eliminate such hazards.  Employees were using an extension ladder to access an upper loft area and the ladder was not set up properly which exposed employees to potential fall hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) Jobsite, 7330 McClure Avenue, Pittsburgh, PA:  On or about April 4, 2019 employees working on the loft floor area with unprotected sides and edges were not protected from falling approximately 13 feet to the lower floor by guardrail systems, safety nets or personal fall arrest systems.

The fall hazard from the work area to the ground below was approximately 26 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Jobsite, 7330 McClure Avenue, Pittsburgh, PA: On or about April 4, 2019,  a twenty-four foot extension ladder was being used to access the loft area from the floor below and did not extend at least 3 feet above the upper landing surface being accessed.
