29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about April 3, 2019, the employer did not ensure each employee, working in an area where new waterline pipes were being installed, was trained in the recognition and avoidance of being in a struck-by zone of the pipe. This exposed the employees to a struck-by hazard.
