29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  JOB SITE: 638 Sundance Drive, O Fallon, MO 63368, on or about 4/3/2019, workers were observed were not protected by protective helmets (hard hats) while working in areas where there was a possible danger of head injury from impact.  A worker was throwing up material to a worker above on a pump jack scaffold and later was cleaning and picking up debris underneath a second pump jack scaffold with workers on board.  The workers were exposed to a struck-by hazard.

29 CFR 1926.451(g)(1)(vii): 29 CFR 1926.451(g)(1)(vii): Each employee on a pump jack scaffold more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  JOB SITE: 638 Sundance Drive, O Fallon, MO 63368, on or about 4/3/2019, workers were observed up on pump jack scaffold, approximately 12' 4" above ground level, actively installing siding, and were not protected from falls by the use of personal fall arrest systems or guardrail systems.  The workers were exposed to a fall hazard.
