One of the employees was not wearing a hard hat. 29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service: location: front entrance area on or about: 4/19/19 a) A daily inspection of the sling and all fastenings and attachments was not done by a competent person. A hook, welded to a Volvo excavator, was being used while installing granite curbing. The hook did not have a safety latch. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
