29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On April 4, 2019; at least two employees were on a steep-sloped roof, installing tarpaper and shingles at 10624 Capistrano Road, Frisco, TX approximately twenty-seven (27) feet above the ground below and were not protected from a fall hazard by using either guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed; the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a) On April 4, 2019; at least two employees were observed using a Werner aluminum extension ladder placed at the garage at a worksite located at 10624 Capistrano Road, Frisco, TX, which was only approximately one foot above the upper landing thereby exposing the employees using it to a fall hazard of approximately ten feet to the ground below.  b) On April 4, 2019; at least two employees were observed using an aluminum extension ladder placed at the front porch at a worksite located at 10624 Capistrano Road, Frisco, TX, which was only approximately one foot above the upper landing thereby exposing the employees using it to a fall hazard of approximately ten feet to the ground below.   c) On April 4, 2019; at least two employees were observed using an aluminum extension ladder; placed on top of the front awning, at a worksite located at 10624 Capistrano Road, Frisco, TX, which was only approximately one foot above the upper landing thereby exposing the employees using it to a fall hazard of approximately twenty (20) feet to the ground below.
