29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    location: throughout site  on or about: 7/17/19    a) An employee, exposed to a fall hazard while engaged in roofing work, was not provided with fall protection training.        NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail,  was not provided to assist employees in mounting and dismounting the ladder:      location: access between main roof and canopy roof  on or about: 4/4/19      a) Employees were using an 8 ft. ladder for access between the main roof and the canopy roof.  The ladder only extended above the canopy approximately 1 foot.      NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

The ladder was used in the leaning position. The spreaders were not opened up.      NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
