Section (K) states the employees was away from work for 5 days, however section (L) does not note work restrictions put on employee.     (b)Case No. R222566 on or about March 4, 2019 a person supported fell on an employee resulting in a knee injury. Section (F) does not specify the parts of the body affected and the object/substance that directly injured or made the person ill.  This injury resulted in time away from work and/or restrictive transfer or job restrictions. Sections (k) and/or (L) do not record any entries.     (c)Case No. R226119 on or about April 2,2019 an employee suffered a knee injury when participating in PCM training. This injury is listed as a sprained knee in section (F), however the injury was a torn meniscus of the left knee. This injury result time away from work and/or restrictive transfer or job restrictions. Sections (k) and/or (L) do not record any entries.    (d) Case No. R226002 on or about April 3, 2019 an employee fell through a window well grate and suffered fractured vertebra and ribs. This incident occurred on the exterior of the building over a window well near the North side of the main entrance of the facility. Section (E) stated the event occurred at LifeScape School Dorm.
