29 CFR 1926.454(a): The employer shall have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   Location: entire site On or about: 4/4/19  a) Employees were using a Genie Z-60 aerial lift to access the roof. One of the employees was not provided with aerial lift safety training.   NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   Location: beside exterior of the building, southwest corner On or about: 4/4/19  a) Employees were using a Genie Z-60 aerial lift to access the roof. The employees were not tied off using a body belt or harness. The employees were exposed to a fall of 18'6".   NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
