29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: This violation occurred on April 2, 2019 where at least two (2) employees worked from an elevated work platform that was not specifically designed or approved by the manufacturer. Employees were exposed to a fall of more than 12 feet to the concrete below.

29 CFR 1926.451(g)(4)(i): Employer did not ensure that guardrail systems are installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold was released for use by employees other than erection/dismantling crews. This violation occurred on April 2, 2019 where at least two (2) employees worked from an elevated work platform that was not equipped with a guardrail system. Employees were exposed to a fall of more than 12 feet to the concrete below.
