29 CFR 1904.30(a): The employer did not keep a separate OSHA 300 Log for each establishment that was expected to be in operation for one year or longer:  COMPANY-WIDE:  On or about and at times prior to December 27, 2018:  1.  All injuries and illnesses at company locations were recorded on one OSHA 300 log, regardless of the physical address where the employee was assigned.  Injuries/illnesses that occurred at facilities at five distinct addresses in New London, CT; Waterford, CT; Groton, CT; and East Lyme, CT were recorded on the 2018 300 log.    2.  Column "F" of the 300 logs was populated with only diagnosis codes indicating the specific illness or the parts of the body that were injured.  There was no description of how the illnesses/injuries were sustained.  Recording of injuries and illnesses in this manner materially impeded OSHA's ability to assess the safety and health of the workplace.

29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year.  COMPANY-WIDE:  On or about and at times prior to April 24, 2019, the employer had not completed a thorough review of the 2018 OSHA 300 log to ensure that the log was accurate and complete.  Several of the entries made in 2018 were for incidents that happened at locations other than Lawrence + Memorial Hospital, 365 Montauk Avenue, New London, CT.  As a result, the OSHA 300A was not correct.  Recording of injuries and illnesses in this manner materially impaired OSHA's ability to assess the health and safety of the facility.
