29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a. Rear, Ground Floor, Mudroom, 174 Hillcrest Avenue, Philadelphia, PA 19118 - The employer did not cover a floor hole that measured approximately 3.7 feet wide by 4.5 feet long. Employees were exposed to a fall hazard of approximately 7 to 8 feet onto a concrete floor. Observed On or About 04/03/19
