On or about April 5, 2018, at the above addressed jobsite, employees performing residential guardrail and stair-rail operations were exposed to falls of thirteen (13) feet, eleven (11) feet five (5) inches, and eight (8) feet four (4) inches (X2) from unguarded floor openings, thereby exposing employees to fall hazards.
