29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection were not provided by placing and keeping such materials or equipment at least 2-feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to present materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: This violation occurred on or about April 5, 2019, at the job site located at Jackson and market street, Dallas, TX; where employees were working inside a trench and not protected from tools and equipment falling inside the trench: (a) Kobelco Acera, 140 SR E088 excavator. (b) Hand tools: Leveler, Socket wrench, combination wrench, and a red colored wrench. (c) Loose red dirt not pushed back at least 2-feet from the edge of the trench.
