29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) South wall of the File Room: The 120 volt "GE" relocatable power tap designed and listed to be relocatable was found to be securely attached to the wall above a table with hidden screws, or industrial strength glue rendering it non-relocatable. b) East wall of the Break Room: A 120 volt Fellows relocatable power tap designed and listed to provide power to high concentrations of low powered loads such as computers, printers, radios, etc. was instead used to provide power to high powered equipment such as a Tappan microwave oven, and a Keurig coffee maker. ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.304(b)(2): Outlet devices had an ampere rating less than the load being served: a) East end of the Break Room: A 120 volt Fellows power tap was used to provide power to a Tappan microwave oven (11.5 amps), a Keurig coffee maker (8.3 amps) a total of 19.8 amps, with is a larger amperage than the power taps' 15 amps. ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv): a) East wall of the Break Room: A 120 volt extension cord was found to be plugged into a receptacle outlet in the building. The cord was placed through a 2-inch in diameter hole cut into the wall (constructed of drywall) where it provided a power source to the Evidence Room located directly on the other side of the wall. b) East wall of the Break Room: A 120 volt extension cord running from a receptacle outlet to the other side of the wall to the Evidence Room was used to provide power in lieu of fixed wiring. ABATEMENT CERTIFICATION IS REQUIRED.
