29 CFR 1910.147(f)(4): During shift or personnel changes, specific procedures were not utilized to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout devices between off-going and oncoming employees: a) On or about April 5, 2019, at the Service and Maintenance Shop, employees left their assigned personal locking device keys for trucks under repair process on top of the Fleet Manager's desk during shift changes, exposing employees to potential unexpected energization or start-up of the machine or equipment, or the release of stored energy.
