29 CFR 1926.451(b)(3): Except as provided in paragraphs (b)(3)(i) and (ii) of this section, the front edge of all platforms shall not be more than 14 inches (36 cm) from the face of the work, unless guardrail systems are erected along the front edge and/or personal fall arrest systems are used in accordance with paragraph (g) of this section to protect employees from falling.  a) Front of the scaffold- Employees were exposed to a fall of up to 30ft while working on a pump jack scaffold that had the front edge platforms approximately over 20 inches from the face of the building; no guardrail systems were erected along the front edge and/ or personal fall arrest systems used.    Violation observed on or about 4/8/19.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Front of house- Employees were exposed to a fall of approximately 30ft while working on a pump jack scaffold without the use of a fall protection system.   Violation observed on or about 4/8/19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Front of the house: Employees were exposed to fall hazards of approximately 10 feet while accessing a residential roof. Employees access the roof in order to set up and climb on a scaffold while it was lowered.    Violation observed on or about 4/8/19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Front of the house: Employees were exposed to fall hazards of approximately 10 feet while using accessing a ladder that did not extend 3 feet over the landing.  Violation observed on or about 4/8/19.
