The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done as required by 29 CFR 1910.147(d)(1) through (d)(6):  a. On or about April 9, 2019 and at times thereto, the employer did not ensure that employees performing service and maintenance work on the Line 2 scales and buckets, such as, but not limited to set up activities and re-aligning the center plates, were controlling the electrical energy source that exposed employees to a caught-in hazard. The energy control application steps were not applied to the electrical disconnect [per the 1910.147(d)(4-6) requirements]. As a result, actuation of the buckets exposed employees to pinch points.
