Panel boards mounted in cabinets, cutout boxes, or enclosures designed for the purpose and were not dead front.   a) Fort Knox Army Base - Building 297 located at 127 Gold Vault Road in Fort Knox, Kentucky - 1st floor - main hallway at the east wing entrance: The power panel cabinet supplying power to the job site was not dead front exposing workers to electrical shock hazards.

29 CFR 1926.404(f)(1)(iv)(A): AC systems, 50 volts to 1000 volts. AC systems of 50 volts to 1000 volts were not grounded if the system can be so grounded that the maximum voltage to ground on the ungrounded conductors does not exceed 150 volts:   a) Fort Knox Army Base - Building 297 located at 127 Gold Vault Road in Fort Knox, Kentucky - 1st floor - main hallway at the east wing entrance: On or about 4/5/2019, the two GFCI receptacles on the left side of each quad receptacle had an open ground.    b) Fort Knox Army Base - Building 297 located at 127 Gold Vault Road in Fort Knox, Kentucky - 1st floor - main hallway at the west wing entrance: On or about 4/5/2019, the two GFCI receptacles on the left side of each quad receptacle had an open ground.
