29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.   a) Fort Knox Army Base - Building 1118 Saber & Quill - Right back corner from the main entrance in the ball room: on or about 4/17/2019, the charging plug of the Sky Jack II 4626 Scissor Lift was not grounded exposing workers to electric shock hazard.

29 CFR 1926.451(g)(4)(iv): When midrails are used, employer did not ensure chain was installed at a height approximately midway between the top edge of the guardrail system and the platform surface.  a) Fort Knox Army Base - Building 1118 - Saber and Quill job site: On or about 4/17/2019, the employer did not ensure the mid-rail height of the entry/exit gate on the Sky Jack II 4626 Scissor Lift was mid way between the top rail and the floor of the platform.
