29 CFR 1926.300(b)(1): Power operated tools that were designed to accommodate guards were not equipped with the guards when they were in use, to protect the operator and employees nearby from hazards such as ingoing nip point, rotating parts, flying chips, or sparks:     a)  Employer did not provide a blade guard for the Makita 9557 NB grinder being used with a metal blade.  Employees using the grinder to cut bricks were exposed to laceration hazard.     Location: 772 48th Street, Brooklyn NY    On or about 4/3/2019    NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.405(g)(2)(iii): The splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:    a)  Two electric extension cords were repaired/ spliced without attaching/ taping the wires in a secure manner; employees using the cords during construction activities were exposed to electric shock hazard, 110v. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws;     a) Electric wiring was not secured to keep wire connections from pulling part; employees are exposed to electric shock hazard 110v.  --- The power cord outer sheath was pulled from its attachment fitting on the electric cement mixer. --- Two extension cord splice connections were not secured.
