29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about April 9, 2019, employees were exposed to a fall of approximately 17 feet 5 inches while performing roofing activities on a commercial pole barn located at 300 N. Abbe Road, Sheffield Lake, OH 44054. The employees were working from the fork attachments of a SkyTrak 6036 Turbo Telehandler. The employer failed to install fall protection for the working platform.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    On or about April 9, 2019, employees were exposed to a fall of approximately 17 feet 5 inches while performing roofing activities on a commercial pole barn located at 300 N. Abbe Road, Sheffield Lake, OH 44054. The roof had a 4/12 pitch. The employees were installing metal roofing and the employer did not ensure they used a means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about April 9, 2019, the employer failed to ensure that extension ladders to gain access to the roof were extended at least 3 feet above the landing surface. Two ladders were used on site to gain access to the roof.
