29 CFR 1910.147(f)(3): 29 CFR 1910.147(f)(3): When servicing and/or maintenance was performed by a crew, craft, department or other group, the employer did not ensure that employee(s) utilized a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device: Employees were exposed to amputation hazards. Two or more employees were unjamming the American Bale Press attached to Weber Bag Machine # 9. Employees did not utilize group lockout tagout. On or about April 1, 2019, an employee received an amputation when the American Bale Press was activated and was not locked out.
