29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about March 25, 2019: On March 25, 2019 an employee stepping down from the bumper of a flatbed truck (22 inches from bumper to ground) slipped and broke his right tibia and fibula and was hospitalized from March 25 to March 28, 2019. The employer did not report the hospitalization until April 3, 2019 at 9:47am.
