29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not in use:      a) Job site, 917 Western Avenue, Pittsburgh, PA: On or about March 19, 2019, during construction activities, the employer did not ensure that a safe means of accessing the scaffolding was provided and used in that an employee climbed up/down the scaffold end frames on a walk through scaffold to gain access to/egress from the scaffold.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   a) Job site, 917 Western Avenue, Pittsburgh, PA: On or about March 19, 2019, during construction activities, the employer did not ensure that appropriate scaffold training was conducted for employees. Deficiencies noted during the inspection included, but was not limited to, failing to use fall protection systems/devices where employees worked on a scaffold where fall hazards of up to at least 10 feet, 1 inch existed, baseplates were not provided as needed and a safe means of access/egress to and from the scaffold was not provided/used.

29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a) Job site, 917 Western Avenue, Pittsburgh, PA: On or about March 19, 2019, during construction activities, the employer did not ensure that an appropriate system was implemented to identify and correct safety and health concerns at the job site. Deficiencies noted during the inspection included, but were not limited to, outlets with no faceplates, electrical cords/equipment exposed to damage and unsafe scaffold practices being followed.
