29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)    On or about 4/12/2019, Furnace pours were being made into the mobile ladles that were moved along the crane rail by crane operators whose view was blocked by the furnace during the pouring cycle rotation exposing furnace tenders working the deck to potential  struck by hazards when the furnace was lowered back into its home position.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a)    On or about 4/12/2019, The bench grinder that was available for Melt maintenance employees to use had the tongue guard out of adjustment as the clearance between tongue guard and wheel was approximately 1 1/2". The grinder is used to touch up chisels after they are cut off.
