29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:     On or about April 10, 2019, during the installation of roofing material at a job site located at 5695 Essex Dr, Seven Hills, Ohio 44131; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces of approximately 9.2 feet above the ground.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.     On or about April 10, 2019, during the installation of roofing material at the job site located at 5695 Essex Dr, Seven Hills, Ohio 44131; the employer did not ensure employees working on the roof of a residential location were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about April 10, 2019, during the installation of roofing material at a job site located at 5695 Essex Dr, Seven Hills, Ohio 44131; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 9.2 feet by utilizing a fall protection system.       The GEORGE'S QUALITY ROOFING LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1330113, citation number 01, item number 01 and was affirmed as a final order on 8/8/2018, with respect to a workplace located at 18261 Martians Lane, Strongsville, Ohio 44149.
