29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about April 5, 2019, the employer did not ensure that procedures for the control of hazardous energy were utilized by employees conducting setup activates on the Mazak CNC machine No. 65. Employees were exposed to a caught in or struck by hazard while conducting setup actives on Mazak CNC machine No. 65 without first locking the machinery out.

29 CFR 1910.147(c)(7)(i)(A): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about April 5, 2019, the employer did not ensure that employees working as setup machinists were trained as authorized employees in the proper lockout tag-out procedures for isolating the Mazak CNC machine No. 65. Employees were exposed to the hazard of startup while conducting setup activities on Mazak CNC machine No. 65 in that the machine's energy sources were not lockout.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence. a) On April 5, 2019, employees who performed servicing and maintenance on and around the Mazak CNC machine No. 65 were not required to: (1) know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy, (2) Turn off and shut down the machines, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device, (5) prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished. Employees required to rely upon a Euchner safety switch were exposed to a struck-by hazard from unexpected start-up of the Mazak VCN 700D CNC Machine No. 65.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips, and sparks: a) On or about March 26, 2019, an employee reached into and contacted the cutter of the Mazak CNC machine No. 26 located on the production floor during the production of aluminum C Pillars used in JLTV trucks. The machine was not guarded, the Euchner Mechanical safety switch was inoperable in that the doors of the machine were able to remain open during production. b) On or about April 10, 2019, an employee working on Mazak CNC machine No. 65 located on the production floor during the production of 700-2-D- matrix controllers was exposed to rotating parts in that the machine was being operated with the Euchner mechanical safety switch bypassed. c) On or about April 10, 2019, an employee working on Mazak CNC machine No. 10 on the production floor during the production of metal latch hinges was exposed to rotating parts in that the machine was being operated unguarded. The Euchner mechanical safety switch was inoperable in that the doors of the machine were able to remain open during production.
