29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: At a jobsite located at 18909 East 48th Street South, Independence, Missouri 64055, an employee engaged in roofing work on a low sloped roof was exposed to serious head injuries. The employee was engaged in hoisting roofing material onto the roof utilizing a Manitex 2892C crane. The employee was not protected by a protective helmet in an area where there was a possible danger of head injuries from falling roofing material from the crane.

The employees were engaged in hoisting operations while utilizing a Manitex 2892C crane. The employees were not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems. The employees were exposed to a 14 foot 10 inch fall hazard to the concrete ground below.

29 CFR 1926.502(h)(2): Mechanical equipment was used or stored in an area where a safety monitoring system was used to protect employees were involved in roofing operations: At a jobsite located at 18909 East 48th Street South, Independence, Missouri 64055, employees engaged in roofing work on a low sloped roof were exposed to serious fall hazards. Employees were engaged in low sloped roofing work while utilizing a Graco Reactor portable electric foam sprayer. This equipment was powered by a portable Honda generator. Both pieces of mechanical equipment were located on a wheeled cart and were being used in an area where a safety monitoring system was used to protect employees. Employees were exposed to a 14 foot 10 inch fall to the concrete ground below.
