29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  On or about April 10, 2019 and at times prior thereto, employees were working on the top of a grease trap/tank with open sides located inside a below-ground vault at the Austin-Bergstrom International Airport without fall protection, exposing employees to a fall hazard of approximately 8 feet.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces in the workplace:  On or about April 10, 2019, and at times prior thereto, employees entering the below-grade vault to service the grease trap/tank at the Austin-Bergstrom International Airport were not informed of the space being a permit required confined space and the dangers associated with entering it, exposing the employees to inhalation hazards.

29 CFR 1910.146(c)(3): When the employer decided its employees would not enter permit spaces, the employer did not take effective measures to prevent its employees from entering the permit space.  On or about April 10, 2019, and at times prior thereto, employees were entering a permit required confined space to service a grease trap/tank at the Austin-Bergstrom International Airport without effective measures implemented by the employer to prevent entry, exposing employees to a hazardous atmosphere inside the space.
