29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour time-weighted average sound level (TWA) of 85 dBA: In the welder department, where employees were exposed to continuous noise level at 239% of the allowable 8-hour time-weighted average sound level (85 dBA). The equivalent dBA level of 239% is approximately 96.3 dBA. The sampling was performed for 476 minutes during one shift on April 17, 2019.

29 CFR 1910.151(c): Where the eyes and body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body was not provided within the work area for immediate emergency use: In the T Fab area of the manufacturing facility, employees using corrosive chemicals were not provided with an eye wash station within the spray area for immediate flushing of the eyes in an emergency, exposing employees to eye and skin injuries.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a non-combustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: In the maintenance area of the facility, the employer had oxygen cylinders stored with flammable acetylene cylinders and other mixture cylinders exposing employees to fire hazards.
