29 CFR 1910.106(e)(6)(ii): Class I flammable liquid(s) were dispensed into containers without electrically interconnecting the nozzle and the container: On or about April 10, 2019, and at times prior thereto, at the paint areas lacquer thinner having flashpoint of 24 degrees F was dispensed into a smaller container without being effectively grounded and bonded to electrically interconnect the containers. Employees were exposed to flash fire hazards.

29 CFR 1910.110(f)(4)(i): The employer stored LP-Gas in a quantity exceeding 300 pounds (approximately 2550 cubic feet in vapor form) inside of a building: On or about April 9, 2019, or at times prior thereto, the propane storage areas in the shopping/receiving department and foam area had a combined quantity exceeding 300 pounds stored inside the building. Employees were exposed to fire and explosion hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 9, 2019, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances: 1) In the Foam Faster and Non-Woven Conversion departments, wire saws did not have guards provided on the left side where other employees in the area would be exposed to the cutting wire. 2) In the Foam Faster department, a foam saw (chopper) did not have guard provided in the back where other employees can be exposed to the blade while in operation. 3) At the press area, press machines had a guard provided however, the guard can be opened easily by hand without using any special tools where employees can be exposed to point-of-operation while in use. 4) In the Non-Woven department, non-woven machines had conveyor systems with in-going nip points without a guard provided. 5) At the trash compactor areas, employees were allowed to operate a trash compactors which did not have a dead-man switch or a guard with interlock device provided. Employees were exposed to amputation and crushing hazards.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: On or about April 9, 2019, and at times prior thereto, at the wire saw machine employees were allowed to use compressed air nozzles for cleaning purpose which was not the type that would reduce the pressure to below 30 p.s.i. The employees were exposed to skin and eye injury.
