29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    Location: a) Supported scaffold on the southwest side of the first floor of the jobsite:                                 On or about 04/09/19, employees were preparing the steel structure on the second tier platform of a two-tier-high supported scaffold. The platform that the employee working on was not fully planked or decked between front upright and the guardrail supports, and were exposed to falling approximately 12 feet to the lower level.
