29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a)  Roof - Employees were permitted to conduct roofing operations in proximity to a 120/240 volt secondary power line located approximately 3 feet above the roof that was not de-energized and grounded or effectively guarded, on or about April 10, 2019.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.416(a)(3): When and where energized circuits existed, the employer failed to post and maintain proper warning signs, advise employees of the location, hazard involved and the proper protective measures to be taken:    a)  Roof - Employees were permitted to perform roofing operations in proximity to an energized 120/240 volt secondary power line without the employer posting warning signs, advising employees of the location, hazard involved, and proper protective measures to be taken, on or about April 10, 2019.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a)  Roof - Employees were not provided with fall protection while conducting roofing operations on a low slope single story residential roof and were exposed to an approximate 8 foot fall hazard to the ground below, on or about April 10, 2019.    H.W. Wilson Roofing Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1073291, citation number 1, item number 1 and was affirmed as a final order on October 19, 2015, with respect to a workplace located at 2209 Market Street, Harrisburg, Pennsylvania.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
