29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Shop areas: On April 11, 2019, and at times prior, the employer exposed employees to struck-by, crushed-by, and amputation hazards, in that employees were allowed to make adjustments and change tools and dies on the Hyundai Hit 18S CNC Lathe, the Hurco VM2 CNC Lathe, and the Chinhung Manual Lathe, without developing, documenting and utilizing energy control procedures to prevent accidental startup of the machine. b) Shop areas: On April 11, 2019, and at times prior, the employer exposed employees to amputation hazards in that employees were allowed to change the blade of the Do All Metal Master band saw, the Kalamazoo band saw, and the Jet band saw without developing, documenting and utilizing energy control procedures to prevent accidental startup of the machine.

The employer failed to maintain a list of the hazardous chemicals known to be present at the facility in accordance with the regulation.
