29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a)   2564 Camas Lane East Petersburg, PA - The employer used a Pettibone 6044 Serial# (3249) material handler to support a scaffold platform at the front of the residential home to adjust a roof truss that was not designed by the manufacturer for that purpose, thereby exposing employees to injuries on or about April 8, 2019.  b)  2564 Camas Lane East Petersburg, PA - The employer used a Pettibone 6044 Serial# (219 ) material handler to support a scaffold platform at the back of the residential home to install plywood sheathing that was not designed by the manufacturer for that purpose, thereby exposing employees to injuries on or about April 9, 2019.  c)  2564 Camas Lane East Petersburg, PA - The employer used a Pettibone 6044 Serial# (219 ) material handler to support a scaffold platform at the front of the residential home to install blocking in the roof trusses and plywood sheathing the platform was not designed by the manufacturer for that purpose, thereby exposing employees to injuries on or about April 9, 2019.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a) 2564 Camas Lane East Petersburg, PA - The employee was walking the top plate board on the back of the building adjusting a roof trusses using an air gun to nail it in on a residential home without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 26 feet to the ground below, on or about April 8, 2019.  b) 2564 Camas Lane East Petersburg, PA - An employee was on the back roof, attaching sheathing on a residential home without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 26 feet to the ground below, on or about April 9, 2019.  b) 2564 Camas Lane East Petersburg, PA - Employee(s) were up in the roof trusses using an air gun to nail install extra blocking on the roof trusses and installing plywood on a residential homes roof without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 19 feet to the ground below, on or about April 9, 2019. The operator had dismounted the Pettibone 6044 material handler leaving no one at the controls, exposing the employees to injuries on or about April 8, 2019.  b) 2564 Camas Lane East Petersburg, PA - Employees were observed working from an elevated work platform at the front of the house between the garages installing roof truss blocking and plywood from a Pettibone 6044 material handler. The operator had dismounted the Pettibone 6044 Serial #3249 material handler leaving no one at the controls, exposing the employees to injuries on or about April 9, 2019.  c) 2564 Camas Lane East Petersburg, PA - An employees was observed working from an elevated work platform at the back on the house installing roofing sheathing on the forks of a Pettibone 6044 Serial #219 material handler. The operator had dismounted the Pettibone leaving no one at the controls, exposing the employees to injuries on or about April 9, 2019.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
