29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) 100 Outlook lane - An employee sustained an injury and was taken to Chambersburg hospital on March 4 2019 for a finger tip amputation. The employer did not report finger tip amputation to OSHA until April 11 2019. No abatement certification or documentation required.
