29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosure: a) The circuit breaker panel at the jobsite entrance did not have an inner shield/cover; live electric parts were exposed. Construction personnel using the panel to turn on/off the jobsite power and lights were exposed to electric shock hazard. Location: 1926 Ave X, Brooklyn NY On or about 4/10/2019 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.760(a)(2): On multi-story structures, perimeter safety cables were not installed at the final interior and exterior perimeters of the floors as soon as the metal decking has been installed: a) Fall protection cables were not installed at the floor edge, exposing employees to 40 ft fall hazard; cables were 10 feet back from the edge at two corner locations at the back of the building, 3fl and 4fl. Location: 1926 Ave X, Brooklyn NY On or about 4/10/2019 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
