29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.        At the jobsite, employees used a red extension cord supplying power to a Circular Saw where the strain/tension pulled the outer protective sheathing away, exposing employees to electrical shock and fire hazards.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.      At the jobsite, employees were not wearing hardhats while working below other employees that were installing framing material to a residential house, exposing employees to struck-by hazards from falling objects.      Antonio Rico Framing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1276538, Citation Number 1, Item number 1, and was affirmed as a final order on February 21, 2018, with respect to a workplace located at 507 Pineview Drive, Euless, Texas 76039.

At the jobsite, employees operating Hitachi nail guns while performing framing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects. At the jobsite, employees engaged in framing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 10 feet to the ground below.
