29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Warehouse: On April 11, 2019, and at times prior, the employer exposed employees to struck-by hazards in that Forklift (Crown Model RC5545-40 Serial Number 1A419535) had a frayed hydraulic hose located at the mast but was not taken out of service.
