29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a. On or about April 12, 2019, at the above addressed jobsite, an employee was conducting electrical operations in a trench measured at seven (7) feet in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about April12, 2019, at the above addressed jobsite, the portable ladder used to access the trench did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
