29 CFR 1926.501(b)(1):  Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Top of roof bulkhead:  Two employees were building parapet walls with cinder blocks at unprotected edges of the roof bulkhead without fall protection, and were exposed to falling approximately 50 feet to ground. On or about 04/09/19.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan.  a) Stairways between ground floor and the 4th floor:  Employees used a stairway to travel between ground floor and the 4th floor. The treads of pan stairs were not filled or fitted with wood or other solid materials at least to the top edge of each pan to prevent tripping hazard. On or about 04/09/19.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.  a) 3rd floor balcony:  An employee leaned a folded A-frame stepladder against a wall and climbed on the ladder for elevation to remove debris. The stepladder was not designed to be climbed on in the folded form for work elevation. On or about 04/09/19.
