29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: Location - 255 Grand Union Blvd., Somerville, MA, 02145 On or about April,11 2019, the employer had not performed an evaluation or identification of respiratory hazard for chemical hazard, such as, but not limited to, carbon monoxide, while operating powered washer machine in an enclosed area. (a) An employee was exposed to carbon monoxide (CO), during cleaning of fire residues. The level of the carbon monoxide (CO) was at an 8-hour time weighted average (TWA) of 114.7 ppm as determined by the employee's Carboxyhemoglobin (%COHb), which was 2.29 times the permissible exposure limit of 50 ppm CO. (b) An employee was exposed to carbon monoxide (CO), during cleaning of fire residues. The level of the carbon monoxide (CO) was at an 8-hour time weighted average (TWA) of 85.2 ppm as determined by the employee's Carboxyhemoglobin (%COHb), which was 1.70 times the permissible exposure limit of 50 ppm CO. 29 CFR 1910.1000(a)(2): Other substances -- 8-hour Time Weighted Averages. An employee's exposure to any substance in Table Z-1, the exposure limit of which is not preceded by a "C", shall not exceed the 8-hour Time Weighted Average given for that substance any 8-hour work shift of a 40-hour work week.
